Facing tongue cancer surgery is frightening โ the right surgeon changes everything
Being told you need an operation for tongue cancer is overwhelming. The questions come fast: Is it curable? Will I be able to talk and eat afterwards? Who should do my surgery โ and what will it cost? You do not have to face those questions alone.
Tongue cancer is highly treatable when caught early โ and because the tongue reaches the neck lymph nodes early, prompt surgery usually means removing less tongue while curing more. Every week of delay lets the cancer grow and forces a bigger operation. Start with an accurate diagnosis of oral and neck cancer at a dedicated oral cancer hospital in Ahmedabad.
See a specialist soon if you have:
- A tongue ulcer or sore that has not healed in 3 weeks
- A lump, thickening or hard patch on the side of the tongue
- A red or white patch on the tongue that keeps coming back
- Pain in the tongue or ear when swallowing or talking, or a change in speech
- A lump in the neck, or a biopsy that mentions tongue cancer
What is tongue cancer surgery?
Tongue cancer surgery removes the cancer from the tongue with a margin of healthy tissue around it, and usually checks the neck lymph nodes for spread. The operation to remove part or all of the tongue is called a glossectomy, and it is the primary, most effective treatment for cancer of the oral (mobile) tongue. How much tongue is removed depends on the size and position of the tumour; here are the main types.
๐
Partial glossectomy
Removal of less than half the tongue, for small early tumours. Speech and swallowing usually recover very well, and many patients need only a simple closure rather than a flap.
Hemiglossectomy
Removal of one half of the tongue, on the side of the tumour, with same-sitting reconstruction to restore the tongue’s bulk and movement.
Subtotal glossectomy
Removal of more than half the tongue, for larger tumours that cross the midline or reach the base of the tongue. Free-flap reconstruction is essential here to keep swallowing possible.
Total glossectomy
Removal of the whole tongue in advanced disease โ done only when it is genuinely the safest option, always with reconstruction and an intensive rehabilitation plan.
Neck dissection
Removal and testing of the lymph nodes in the neck, done alongside the tongue surgery. Selective techniques remove only the node levels at risk, protecting the nerves that move the shoulder and lip.
Composite & salvage surgery
When the cancer reaches the floor of the mouth or jaw, or has returned after earlier treatment, tongue, bone and neck are treated together โ see surgery for advanced oral & neck cancer.
Tongue cancer surgery success rate: what decides it
Every family asks this first. There is no single number that fits everyone โ the tongue cancer surgery success rate depends on your particular cancer, and two patients with the same diagnosis can have very different outlooks. What we can tell you is exactly which factors move the odds, because most are still in your control today.
The stage at which you operate
The single biggest factor. Caught and operated early โ while the tumour is small and the neck is clear โ tongue cancer has a very high chance of cure and often needs no further treatment. Nothing improves your odds as much as not waiting.
Whether the margins are clear
Success depends on removing the cancer with a clean margin of healthy tissue all around it, confirmed by the pathologist. Cancer left at the edge is the commonest reason a tongue cancer returns.
Whether the neck nodes are involved
Whether the cancer has reached the lymph nodes strongly influences the outcome โ which is why the neck is tested during the same surgery, so the rest of your treatment can be planned correctly.
Who performs the surgery
Complete clearance, safe neck dissection and same-sitting reconstruction need a surgeon trained specifically in oral and head-and-neck oncology. Getting the first operation right matters more than any operation that follows.
Completing the treatment that follows
Finishing any advised radiotherapy or chemotherapy, stopping tobacco, gutka, mawa and alcohol completely, and attending every follow-up all protect your result and catch a recurrence early.
Want an honest read on your case? We will review your biopsy and scans and tell you what a realistic outcome looks like for you.
Tongue cancer surgery reconstruction: speaking & eating again
โWill I be able to talk and eat if part of my tongue is removed?โ is the question almost every patient asks first โ and the answer depends on whether the tongue is rebuilt at the time it is operated. That is why reconstruction is planned in from the first consultation, never left as an afterthought.
- Same-sitting microvascular free-flap reconstruction rebuilds the tongue during the same operation โ a radial forearm flap for fine movement, or an anterolateral thigh flap for more bulk.
- Function-first planning restores the tongue’s bulk and mobility, backed by our wider reconstructive & oncoplastic surgery.
- Speech, swallowing & dental & functional rehabilitation help you return to talking and eating with confidence.
- Honest, compassionate guidance โ you will always know what to expect before the operation happens.
Tongue cancer surgery cost in India
One of the first questions families ask is what the surgery will cost. There is no single fixed price โ the tongue cancer surgery cost depends on your specific case. In India, this surgery costs far less than in Western countries, and we work to keep expert care genuinely affordable. Rather than a generic price list, we give you a clear, personalised cost estimate after your consultation.
What affects the cost of your surgery
- How much tongue is removed โ a partial glossectomy is a very different operation from a subtotal or total glossectomy.
- Reconstruction needed โ a primary closure versus microvascular free-flap reconstruction, which adds surgical time and expertise.
- Neck dissection โ whether the neck nodes are removed, and how many levels are involved.
- Hospital stay & care โ length of admission and level of post-operative care required.
- Additional treatment โ whether radiation or chemotherapy is needed after surgery.
- Diagnostics โ biopsy, CT/MRI/PET scans and staging done before the operation.

Tongue cancer surgery โ frequently asked questions
Will I be able to speak and eat normally after tongue cancer surgery?
In most cases, yes โ provided the tongue is reconstructed at the time of surgery. For small early tumours removed by partial glossectomy, speech and swallowing usually return close to normal. For larger removals, tongue cancer surgery reconstruction using a free flap from the forearm or thigh restores the bulk and movement needed to speak and swallow, and speech therapy afterwards helps you regain function over the following months. Recovery is gradual, and it is faster when the cancer is treated early.
What is the tongue cancer surgery success rate?
There is no single figure that applies to every patient, and we will not put one on a web page. The tongue cancer surgery success rate in your case depends mainly on the stage at which you operate, whether the margins around the tumour come back clear, whether the lymph nodes in the neck are involved, the experience of the surgeon, and whether you complete any treatment advised afterwards. Early-stage tongue cancer treated promptly with complete surgery has a very good chance of cure. At your consultation we will review your reports and give you an honest, specific answer for your own situation.
How much does tongue cancer surgery cost in India?
The tongue cancer surgery cost in India varies widely depending on how much of the tongue is removed, whether a free-flap reconstruction and neck dissection are needed, your hospital stay, and whether radiotherapy or chemotherapy follows. Because of this, we give a personalised written estimate rather than a fixed price. Send your biopsy report and scans on WhatsApp, or bring them to a free consultation, and you will get an itemised estimate at no cost. Most health insurance policies cover cancer surgery, and our team will help with the pre-authorisation paperwork. โ ๏ธ confirm insurance assistance.
How long does the operation take and how long will I be in hospital?
A small partial glossectomy may take a couple of hours, while a larger glossectomy with neck dissection and free-flap reconstruction can take considerably longer, as the reconstruction is done in the same sitting. Most patients stay in hospital for about a week so the reconstruction can be monitored and feeding supported while it heals. You will be given a realistic timeline for your own operation before you consent to it.
Is the whole tongue always removed in tongue cancer?
No โ and this is one of the most common fears we correct. Total glossectomy is reserved for advanced disease where it is genuinely the safest option. Most patients need only a partial glossectomy or a hemiglossectomy, removing part of the tongue with a clear margin. How much has to be removed depends on the size and position of the tumour, which is another reason not to delay: earlier surgery usually means less tongue removed.
What is tongue cancer surgery reconstruction, and is it done at the same time?
Reconstruction rebuilds the part of the tongue that was removed, using tissue from your own forearm or thigh connected to the blood vessels of the neck under a microscope. At Heyansh Oral Cancer Hospital it is done in the same operation as the cancer removal, so there is no second surgery and no waiting period โ and because the same surgeon plans both, the removal is designed around what can be rebuilt.
Will I need radiotherapy or chemotherapy after surgery?
Not always. It depends on what the pathology report shows after the operation โ the size of the tumour, how deep it went, whether the margins were clear, and whether the neck nodes were involved. Early cancers removed completely often need no further treatment. If radiotherapy or chemotherapy is advised, it is to reduce the risk of recurrence, and completing it as planned matters.
Can tongue cancer come back after surgery?
It can, which is why follow-up is not optional. Recurrence is more likely if the margins were not clear, if the nodes were involved, or if tobacco, gutka, mawa or alcohol use continues after treatment. A recurrence found early at a routine follow-up is far more treatable than one found late โ so keep every appointment, and come back immediately if you notice a new ulcer, lump or change in speech.
I have been told my tongue cancer is inoperable. Is that final?
Not necessarily. Cases are sometimes assessed as inoperable without a reconstructive surgeon involved, and reconstruction can change what is surgically possible. We accept advanced and recurrent cases and offer a free second opinion โ bring your biopsy report and scans. If we agree that surgery is not the right option for you, we will say so honestly and help you understand what is.
